Welcome to the practice!  We look forward to seeing you and giving you a result that will help you look and feel your best. Please review our scheduling policy. If you have any questions, please give us a call and someone will assist you and provide an explanation of our full policy and procedures.

Cosmetic Patients

You will receive an e-mail with instructions to register for our patient portal upon scheduling an appointment. We highly encourage our patients to complete their new patient forms on their portal. If you are unable to do so, please come to the office at least 15 minutes early if you need to complete our patient information forms.
There are fees for surgical consultations, ranging from $50 – $300 based on the type and complexity of your appointment. The consultation fee is refundable if we have 24 hours notice for cancellations or reschedules. If you do not show up to your appointment or give us less than 24 hours notice to cancel, a new consultation fee will be collected. Patients who have had surgery with another doctor and have a problem that requires immediate attention need to call the office to discuss the nature of their problem. A revisional consultation fee will apply in order to be seen.

Cosmetic Laser/Injectable Patients

A deposit is required to schedule any cosmetic office treatment if it is not performed on the day of consultation (e.g. laser treatment, botox/fillers, cosmetic procedures). This will be applied to your treatment but is nonrefundable for no-shows or cancellations with less than 24 hours notice. We value your time, and procedures/ laser treatments require set-up time prior to your treatment. Patients later than 10 minutes to their appointment are considered no-shows, and your $50 deposit will be forfeited. Please call us if you are running late, we will try to accommodate your treatment if our schedule allows.

Insurance Patients

We request that all medical history and insurance forms be completed online prior to your visit through your patient portal. You will be given a link to register upon scheduling your appointment. If you are unable to do so, please arrive 15 minutes early to your scheduled appointment. Paper forms are also located at the bottom of this webpage and may be printed at home and brought to your visit. Please be prepared to provide this information to our office along with current medical insurance information. We require 24 hours notice to cancel or reschedule your appointment. If you need to reschedule within that window or do not show up to your appointment, there will be a fee of $50 that must be paid prior to rescheduling. Your copay will be collected at the time of your visit. If you cannot pay your copay at the time of your visit, your appointment will be rescheduled.
