Please come to the office at least 15 minutes early if you need to complete our patient information forms.
There is a $50 fee to schedule a cosmetic consultation. This will be applied to any treatment you may have within 6 months of consultation but is nonrefundable with less than 24 hours’ notice to cancel your appointment and no-shows. Patients who have had surgery with another doctor and have a problem that requires immediate attention need to call the office to discuss the nature of their problem; a revisional consultation fee will apply in order to be seen.
If you have had prior cosmetic consultation with us, subsequent cosmetic consultations will be complimentary. Existing patients, no-shows, and cancellations within 24 hours’ notice to cancel will have the consultation fee reapplied to reschedule.
Cosmetic Laser Patients
A $50 deposit is required to schedule any cosmetic office treatment if it is not performed on the day of consultation (e.g. laser treatment, injectable, cosmetic procedure). This will be applied to your treatment but is nonrefundable for no-shows or cancellations less than 24 hours notice. We value your time and procedures/ laser treatments require set-up time prior to your treatment. Patients later than 10 minutes to their appointment are considered no-shows and the $50 deposit will be forfeited. We try to accommodate those that are late as long as the schedule allows.

These forms may be emailed to us or printed at home and brought to your visit. Please be prepared to provide this information to our office along with current medical insurance information. We require 24 hours notice to cancel or reschedule your appointment. If you need to reschedule within that window there will be a fee of $50 that must be paid prior to rescheduling. Your copay will be collected at the time of your visit. If you can not pay your copay at the time of your appointment will result in the appointment being canceled or rescheduled.

Please be prepared to provide this information to our office along with your current medical insurance information.
