Out of Network Notice

Please be aware that Dr. Stephen Small is out of network with some commercial insurances. If you have out of network benefits with any of these plans, we will bill your insurance for your visit. You may have a higher out of pocket cost for an out of network visit. Please call the phone number on the back of your card prior to scheduling to find your out of network responsibility. We are currently out of network with the following insurances:

UnitedHealthcare/Oxford
Aetna
Cigna
All forms of Medicaid

In Network Insurances

We do participate with Medicare, commercial Blue Cross/Blue Shield, and Amerihealth. We will bill your insurance for an in-network visit. However, you may still have a financial responsibility in the form of a copay, co-insurance, or deductible. We recommend that you call the number on the back of your card prior to scheduling to find out if any of those will apply to your visit with Dr. Small.
